Cardiology Care at Carteret Health Care Enhanced with Opening of Transitional Clinic

Carteret Health Care is pleased to announce the launch of a new Transitional Clinic for cardiology patients, opening on October 1, 2024. The clinic will provide essential follow-up care for heart failure patients discharged from the hospital, with a focus on reducing rehospitalization rates.  

Upon discharge from the hospital, patients will be scheduled for appointments in the Transitional Clinic. This clinic is designed specifically for high-risk heart failure patients, ensuring they receive a follow-up visit with a cardiologist within seven days of leaving the hospital. The clinic focuses on targeted transitional care, rather than serving as a primary cardiology service for those already under the care of a cardiologist. By addressing the challenge for some patients of securing timely appointments due to scheduling constraints, the Transitional Clinic aims to reduce the risk of rehospitalization for these fragile patients.

Led by Dr. David Jessup, Medical Director of Cardiovascular Services, the clinic enhances Carteret Health Care’s cardiovascular offerings. Under Dr. Jessup’s leadership, patients can expect exceptional care and personalized attention as the transition from hospital to home.

The opening of the Transitional Clinic is a significant step in enhancing our care for heart failure patients, providing them with timely access to follow-up services right after hospital discharge” stated Dr. Jessup.

The Transitional Clinic will also offer short-term follow-up care to bridge the gap between hospital discharge and scheduled cardiology appointments. Newly diagnosed heart failure patients without an established cardiologist will also benefit from this service. This service is not intended to replace the care provided by existing cardiologists or primary care providers.

Located at 3610 Medical Park, Morehead City, the Transitional Clinic is dedicated to supporting patients in managing their health after discharge, streamlining their transition back to everyday life while ensuring access to necessary follow-up services and education. During their visit, patients will receive comprehensive care including:

Evaluation of their overall condition and response to hospital therapy.
Review of medications, with adjustments made as needed.
Help to arrange follow up diagnostic studies related to their medical condition.
Education on their diagnosis, including key symptoms to monitor and contact information for any changes.
Assistance with arranging follow-up appointments with their cardiology team and other specialists.
